Case management is a Client-focused intervention that is regularly reviewed and evaluated to ensure it is a fluid process which continues to work towards the Client’s changing needs and goals. Case management should be non-intrusive and delivered at a pace that the Client and their families are comfortable with.
The Case Manager will endeavour to develop a therapeutic, mutually respectful and trusting relationship with the Client and their family.
The Case Manager will always act in the Client’s best interests and with an awareness of the Mental Capacity Act, involving the Client and their family in the decision making process.
